so I thought the retina doc said wait...

Got a call from the cataract ophthalmologist's scheduling nurse today. As I had thought, the retina doc did say he wanted me to wait on the vitrectomy. And so, in early May I'm just going to have my cataract removed. Then my life as a bionic woman with a serially-numbered plastic lens in each eye will be complete. (^_^)I'll also end up getting bifocals in a month or two, as I won't be able to read anything up close anymore. The doc told me before that they don't have the technology to make lenses work like human lenses do, being able to focus both far and near. My glasses will become thinner (since my distance vision will improve), but I'll have to have bifocals to read.I guess I'm supposed to be upset by becoming an early-30s birfocal lady, but you know what? I'd rather be able to see than not! I've worn glasses since I was around five years old, so they're no big deal to me.In other news, I've been having this weird ache in my left-side back since late last week which spread to the front this weekend. The area just under my ribcage feels tender, like there's a bruise or something, except there's not. I have no idea what this could be. One of my friends suggested it might be a kidney infection. *sigh* Guess this means I'll need to try to get in to see my primary-care doc. I really don't want to see her and hear strange comments about electro-shock therapy to help depression, or her questioning why my antidepressants aren't causing me to magically lose weight...you know, things that have nothing to do with why I'm there. :P more...

eyeball update...

May 2005 in Indianapolis...not only the month for the 500 race, my bi-annual dentist appointment, the release of Star Wars Episode III, and my 29+4th birthday (ha)...but also more than likely the month I'll have a combination cataract-removal and vitrectomy surgery done on my right eye.I'm not really nervous about the cataract removal -- it's the vitrectomy. Those things can have such complications, even themselves causing blindness, and although I trust my surgeon 100%, accidents can happen.So...yeah...eek.I'm also dreading the bills. In some ways it might end up being slightly cheaper than the two separate procedures were last year...since I'll only have to have one anesthesiologist, one operating room, one set of nurses (I hope), one recovery room, etc.Oh well, wish for the best, it's all I can do.... more...

Optical Illusions

A popular dieting strategy is to eat less off small plates, a sort of visual trick. Many people scoff at that, but several new studies back it up. They prove that most times it's our eyes -- not our stomachs -- that decide how much we eat. This is crucial to keep in mind these days, when restaurant portions are designed to fill oversized people. The author of these and numerous other studies on how and why we eat is Brian Wansink, PhD, a professor in both the marketing and nutrition departments at University of Illinois, Champaign. In one recent experiment, he asked 76 adults to estimate how much tomato soup they planned to eat from 24-ounce bowls set before them. The majority responded visually: 55% said they would eat until their bowl was empty... another 26% said they would eat until the soup was half gone... only 19% said they would quit when they were no longer hungry. The researchers then divided subjects into two groups and removed the visual cue -- the size of the bowl -- for one group. By secretly refilling their bowls slowly and persistently through tubes in the bottom, the researchers in effect took away the size of the bowl as a way of helping the subject decide how much to eat. After 20 minutes everyone quit eating, at which time the group with refilling bowls had consumed twice the amount that the other group had. However, when asked to rate how full they were, their answers were the same as those in the other group. In other words, the subjects could have consumed half the amount they did and still have experienced the same satisfaction. Dr. Wansink tells me that visual cues influence virtually all of us in determining how much we eat. Many people deny it, but studies repeatedly show that people eat what's in front of them, using an empty plate -- not a full stomach -- to signal them when to stop. Furthermore, people have visual biases based on the tableware itself. We perceive tall, slim glassware as bigger than same-sized short, squat glasses. For one study, a group of teens poured juice into both types of glasses, but assuming the short glasses were smaller, the kids poured -- and drank -- 70% more from them than they did from the tall glasses. Dr. Wansink's advice for smart diners is this: Control portions by deciding before you start how much food you want to consume, and dish up that amount, no more. Use visual biases to your advantage -- drink from tall slim glasses and eat from salad plates to fool your eyes into thinking you're eating more.

The confusion over full spectrum light bulbs.

Many people are interested in the advantages of using full spectrum fluorescent light bulbs. But there is a lot of confusion out there regarding the subject, so people have a difficult time deciding what to buy.

The most important thing to remember is that full spectrum light bulbs are simply a presentation of light that is close to natural daylight color. It's nothing more than that. There are quite a few companies that claim slight variances in the fluorescent phosphor blend and try to charge high, high prices for it. The truth is there isn't that much difference in various brands or technical advances. Full spectrum light bulbs give off light in a more natural way and makes for better visual acuity and color rendition of objects. That's why retailers like this kind of light. It makes colors stand out while showing a product in the same light that a customer might see it after it leaves the store.

I personally like this kind of light as it is whiter and brighter than other types of incandescent for fluorescent lamps. The contrasts are better and as your eyes age, they need more light and a better contrast to see better. Some people use them for SAD light therapy and that's a method that is prescribed by a doctor. The same fluorescent full spectrum light bulbs that we provide at our company are the same that are used in light boxes for this purpose.

When buying full spectrum light bulbs, you shouldn't be confused. Remember that it's simply a light bulb that is a close simulation to natural daylight. Don't pay outrageous sums of money to get it as most brands are very similar.

Glossary of Optical Terms - Heavyglare.com - Directory of Optical Terms

Glossary of Optical Terms - Heavyglare.com - Directory of Optical Terms: "Polarized Lenses: Light waves traveling freely can vibrate in any direction. When light strikes a horizontal reflecting surface such as water, sand or pavement, it vibrates horizontally creating glare. To the unprotected eye, glare can decrease depth perception., reduce visual acuity and create eye fatigue.

These lenses are great by the water or for driving. A good example of how these lenses work. While driving in your car you often see the glare of the dashboard or if you have a white piece of paper on the dashboard for example you see its reflection off the windshield. Having a polarized lens virtually eliminates this reflection and all of this type of glare (horizontal glare). These lenses are laminated and have a polarizing film in side the lens they are available in gray or brown. They are also available in plastic, polycarbonate or glass."

New York Times branches out

The New York Times editorial board has a hard enough time writing lucid op/ed pieces as it is. Now they decide to start whining about the existing treatments for the "wet" form of the common ocular disease macular degeneration? Let's hope the latest good news on a drug for age-related macular degeneration, the leading cause of blindness in the elderly, holds up better than the last round of optimism. There is clearly a need for better treatments given that the only two drugs currently on the market slow progression of the most-damaging or "wet" form of the disease but do not generally improve a patient's sight.Only a few years ago a small early-stage clinical trial seemed to show that Macugen, one of the currently approved treatments, did improve vision. That set off a stampede of patients seeking the supposed miracle cure from their doctors. But when a larger, late-stage trial was completed, only a small percentage of patients had gotten better and the drug's real value turned out to be in stabilizing the disease or slowing deterioration. Now Genentech, a California-based biotech company, has startled eye experts and the stock markets by announcing that its experimental drug, Lucentis, produced a significant average improvement in patients who received the drug in a late-stage trial. A control group suffered an average decline in visual acuity. Over all, about 95 percent of the patients receiving Lucentis injections maintained or improved their vision compared with only 62 percent of a control group given a sham injection. The results appeared better than those obtained with the existing drugs, although experts need to parse the details to be sure. There are good reasons to wait before cheering too loudly. Too few details have been released to judge how many patients got better and how great their improvement was. The study has yet to be presented at a scientific conference or published in a peer-reviewed journal. And the results stem from only the first year of a two-year trial. Anxious patients will await further analysis to determine whether these latest findings add up to real progress or another dashed expectation. So, according to the nitwits at the Times, stabilizing a patient's vision is not "real progress". Tell that to the millions of people with this disease who because of existing treatments like Macugen still have enough vision to ambulate and take care of themselves. If left untreated, their odds of being able to do so would be significantly less. The usual drivel the NY Times editorial board pens is made enough. But talking out of their butts about scientific issues is that much worse. more...

TV Viewing Distance and Screen Placement in the Home Theater

By Andrew Ghigo
The optimum TV screen size is directly related to the available viewing distance – but there are other factors as well that need to be taken into consideration.
Viewing Distance: Is it just a matter of personal preference?
Sit too close to your big screen TV and you will be able to see the image build-up structure - scanning lines or pixels forming the image – thus distracting your attention and spoiling your home theater experience. Yet, sit too far away, and the impact will be lost.
There are differing opinions on the best way to determine the optimum TV viewing distance for a specific screen size. Just go to the movie theater and you will soon realize that it is all a question of personal preference - some would sit at the very back. Others would go straight to the front row, as they prefer the bigger picture and a wider angle of view, while some would simply choose their seat randomly somewhere in between these two extremes.
The truth is that there are no scientific rules her. This does not mean that there aren't any guidelines that you should follow when planning a big screen purchase or a would-be home theater room.
SMPTE Recommendations and the THX Certification standards:
The Society of Motion Pictures and Television Engineers (SMPTE) recommends that the screen size for home theater use should occupy a 30 degrees field of view - in the horizontal plan - for the audience. Alternatively, the ideal TV viewing distance should be such that the screen width occupies an angle of 30 degrees from the viewing position.
This 30-degrees viewing angle seems to have been accepted by many as the standard in home theater and motion picture viewing.
This SMPTE guideline is also in line with the THX certification standards in that these recommend that the back row of seats should have at least a 26 degrees viewing angle and while recommending an optimum viewing angle of 36 degrees.
It is believed that within these viewing angle limits, the viewer will get better immersed into the action movie itself.
Vision System limitations:
There is also the issue of TV viewing distance based on visual acuity. This does not represent the optimum viewing distance - rather, this relates to the maximum viewing distance beyond which some picture detail will be lost.
Technically speaking, visual acuity is a measure of the eye spatial resolving power and indicates the angular size of the smallest detail that a person visual system can resolve. A person with 20/20 (or 6/6 when expressed in meters) normal vision can resolve a spatial pattern separated by a visual angle of one minute of arc angle i.e. 1/60th of a degree, at the eye when viewed at 20 feet away. Expressed differently, a person with normal 20/20 vision is capable of identifying an object with a height of 1.76mm at 20 feet way.
In terms of TV viewing distances, these represent the point beyond which some of the picture detail will no longer be resolved by the viewer vision system.
So How Does All This Translate In Practical Terms?
A few rules-of-thumb can help put in practice the guidelines detailed above. These rules for viewing distance refer to the screen width rather than the screen diagonal and therefore apply to both 4:3 and 16:9 display formats.
A general rule for the TV viewing distance based on the SMPTE and visual acuity guidelines, is that the nearest TV viewing distance between you and your big screen TV should be limited to approximately twice the screen width (more precise 1.87 x screen width for a subtended angle of 30 degrees), while the furthest distance being no more than five times the width of your screen.
This rule of thumb should give you a fairly good approximation for your TV viewing distance. It does not necessarily represent the ideal home theater viewing distance but rather the limits within which your TV viewing distance should theoretically be out of the trouble zone.
In other words, move closer than twice the screen width size, and the picture scanning lines, pixels and any other video artifacts will become too visibly intrusive - leading to distractions that will spoil your movie watching experience. Move further away than 5 times the screen width and your vision system will no longer be able to resolve all the picture detail.
But…
It is also important to realize that these maximum and minimum viewing distances should be seen in the light of the video signal definition.
A fully resolved high definition TV (1080i, 1920x1080) supports a closer viewing distance than standard analog TV. Thus while twice the screen width would be the ideal TV viewing distance for a HDTV display, it would be a bit too close for standard TV; in the later case, a three times the screen width would be a better option.
Similarly, the five times the screen width as the maximum view distance, while more than adequate for a standard analog TV picture, is a bit too far away for a person to see the fine detail supported by a HDTV picture – a three to four times the screen width represents a more practical limit for the maximum viewing distance in the case of HDTV.
These rules-of-thumb work best with big screen TV sizes in the range 42-inches and over.
When it comes to the use of regular-size standard definition analog TVs in the home theater, i.e. up to 36” / 40” diagonal, the optimum viewing distance range is between 8 feet and 12 feet. TV sets smaller than 36-inches aren't big enough to qualify for Home Theater use; their smaller screen size will not provide the desired impact on the viewer.
Vertical Angle of View & Screen Height:
For optimum viewing, the eyes of the viewer should be level with the center of the screen.
Maximum vertical angle of view: In those home theater set-ups where this is not possible, the SMPTE guidelines suggest that the maximum vertical angle measured at the seated eye height from the front row center seat to the top most part of the projected image should not exceed 35 degrees.
This does not represent the optimum viewing angle but rather the limit beyond which the viewer will be subject to an increased neck strain.
This maximum vertical angle limit is always measured from the front row as this represent the extreme angle of view.
Minimum angle of vision: While there do not appear to be any specific SMPTE or THX guidelines in this respect, yet studies have shown that if the screen size occupies less than 15 degrees of the viewer's vertical field of view, than that image appears small.
Practical Considerations:
In a typical home theater set-up, you do not need to really worry about neither the maximum vertical angle of view, nor about the minimum vertical angle of vision for an effective movie theater experience.
If one were to adhere to the recommended TV viewing distance of twice the screen width (as further detailed above based on the SMPTE guideline of 30 degrees horizontal field of vision), you would automatically be complying with the minimum angle of vision. The reason being that there is a fixed relation between screen height and width in accordance to your home theater screen aspect ratio of either 16:9 or 4:3.
Further more, in a typical home theater setup, it would be very difficult to exceed the maximum vertical angle of view beyond which you will be subject to an increased neck strain.
Taking into account that most home theater rooms are approximately 10 feet (3m) high, the resultant vertical viewing angle is normal well within the maximum of 35 degrees detailed in the SMPTE guidelines – all you have to do is just remain within the twice the screen width guideline for your viewing distance.

Optcial Implants!

Just a short, "what-I'm-currently-thinking" kind of entry.I've been going over the possibility of getting contact lenses. I'm sort of getting sick of relying on glasses most of the time to see straight. What I have is called Myopia (or Short-Sightedness). It's a very common eye disorder and it's genetic so it runs in my family (from my Mother's side). I've made an appointment to go see my optometrist on Thursday, and I can't help but feel excited about getting them. Once I get used to them, I might even get some special tinted pairs or something further down the track. ^_^ I'd like to use lenses as an alternative for glasses until I can get laser surgery.I don't totally mind wearing glasses all that often, this is really just for a bit of variety. Not to mention I'll have perfect vision when wearing sunglasses. ^_^ more...

Fibromyalgia and eyes - part the second

So, having done some research on the internet, I found some interesting things... specifically, I found a very nice article called "What your Eye Doctor should know", which explained most of my symptoms. I won't go into all the details here (if you are interested, go to www.sover.net/~devstar/eye_doc.pdf) but basically (besides Sicca Syndrome (ie, dry eyes, nose, mouth) ), it seems likely that my fluctuating lens prescription is due to "tense" eye muscles, with trigger points in those muscles. Indeed, looking into the corners of my eyes is quite painful, and leaves me disoriented.
Solution: stretchercises :)
Dunno about the plaque though...

Pomegranate news

Int J Food Sci Nutr. 2004 Sep;55(6):511-6Antioxidant activity of selected foodstuffs
The growing interest in the substitution of synthetic food antioxidants by natural antioxidants and in the health implications of antioxidants as nutraceuticals has fostered research on vegetable sources and the screening of raw materials for identifying antioxidants. Plant and plant products have been used as a source of medicine for a long time. Among the more important constituents of edible plant products, low molecular weight antioxidants are the most important species. It is known that consumption of fruits and vegetables is essential for normal health of human beings. The aim of the present work was to evaluate the total antioxidant activity of selected natural food materials by an in vitro method involving the measurement ofoxidation of linoleic acid by fluorimetry. Among the food materials chosen for the present study, pomegranate peel gave the maximum antioxidant activity due to the presence of its high polyphenolic content. At a concentration of 60 ppm, pomegranate peel powder reduced lipid peroxidation by 65% in an in vitro assay.

Haptoglobin Type and Antioxidants

It may soon become important to know one's haptoglobin type. There is very suggestive evidence that about 15% of the population may be in dire need of antioxidant supplementation while about 25% may be harmed by it.
Article: Effect of Vitamin Supplementation varies by Haptoglobin type

From the abstract:
Antioxidant trials have not demonstrated efficacy in slowing cardiovascular disease but could not rule out benefit for specific patient subgroups. Antioxidant therapy reduces LDL oxidizability in haptoglobin 1 allele homozygotes (Hp 1-1), but not in individuals with the haptoglobin 2 allele (Hp 2-1 or Hp 2-2). We therefore hypothesized that haptoglobin type would be predictive of the effect of vitamin therapy on coronary atherosclerosis as assessed by angiography.
RESULTSWe found a significant benefit on the change in MLD with vitamin therapy as compared with placebo in Hp 1-1 subjects (0.079 ± 0.040 mm, P = 0.049). This benefit was more marked in diabetic subjects (0.149 ± 0.064 mm,
P = 0.021). On the other hand, there was a trend toward a more rapid decrease in MLD with vitamin therapy in Hp 2-2 subjects, which was more marked in diabetic subjects (0.128 ± 0.057 mm, P = 0.027). HRT had no effect on these outcomes.

Antibiotic May Limit Effects of Diabetes

HERSHEY, PA -- A Penn State College of Medicine study suggests that a common antibiotic called minocycline may slow or prevent diabetic retinopathy, a complication of diabetes that is the leading cause of blindness in people ages 20 to 74.The mechanism involves microglia - immune system cells that clean up other damaged cells. In diabetics, microglia destroy damaged as well as live cells in the cleanup process, leading to vision loss. The microglia activate due to a buildup of cytokines (protiens which cause inflammation). The antibiotic reduces the inflammation and therefore reduces the microglia's activation and in turn slows the retinal damage.

Excessive Computer Use Hurts Eyes

In a particular study on computer users it was discovered that heavy computer users tended to be "long-sighted" (worse than far sighted) or short-sighted. More importantly, about a third of those users had glaucoma. This makes sense because short-sightedness (aka Myopia) is a risk factor for glaucoma. If you are a heavy computer user, you should get regular eye exams so as to not take any risks. According to the article,

He said three eye tests should be carried out to check for glaucoma: ophthalmoscopy (a visual examination of the optic disc), tonometry (a measurement of pressure in the eye) and perimetry (which checks for visual field anomalies.

Glaucoma is a disease of the eye in which the internal pressure becomes so great that it compresses the nerves and can lead to blindness if unchecked.
